A method of establishing and communicating an augmented electronic health record (EHR) for each one of multiple Patients, wherein: each one of a plurality of Providers creates EHR data which describes each instance of Healthcare delivered by each Provider to each Patient; each Patient receives multiple instances of Healthcare over a period of time; multiple Providers deliver the instances of Healthcare to each Patient over the period of time; each Provider stores on a computer system of that Provider, the EHR data which describes each instance of Healthcare delivered by that Provider to each Patient; a predetermined common standard defines a format used by the computer system of each Provider to describe and store the EHR data created by that Provider; the format to describe and store the EHR data for each Patient utilizes a unique identification for each Patient; the predetermined common standard also defines a protocol used by the computer system of each Provider to communicate EHR data between the computer systems of the Providers using a communication network that interconnects the computer systems of the Providers; the protocol to communicate EHR data utilizes the unique identification of each Patient and a unique identification of each Provider when communicating the EHR data between the computer systems of the Providers; and the computer system of one Provider which has EHR data stored for an identified Patient communicates that stored EHR data to the computer system of a different Provider upon receiving a valid request for that EHR data from the different Provider; and the method comprises: an Aggregator attaining a status of a Provider under the predetermined common standard, the status of a Provider enabling the Aggregator to deliver Healthcare to each Patient under the predetermined common standard on an ongoing basis; interconnecting the computer systems of the Aggregator and each Provider with the communication network to enable communications between the computer systems of the Aggregator and each Provider using the unique identifications of each Provider and a unique identification of the Aggregator; the computer system of the Aggregator communicating a request for the stored EHR data describing each instance of Healthcare delivered to the identified Patient during the period of time by the identified Provider that is not the Aggregator; the computer system of the Provider identified in the request automatically communicating the requested stored EHR data in a reply transmitted to the computer system of the Aggregator in response to the request; the computer system of the Aggregator automatically collecting the EHR data transmitted in each reply and aggregating the collected EHR data with any pre-existing EHR data of any pre-existing augmented EHR of the identified Patient to create an updated augmented EHR for the identified Patient, the updated augmented EHR including the EHR data for each instance of Healthcare delivered to the identified Patient by each identified Provider over the period of time; the computer system of the Aggregator storing the updated augmented EHR in the format of the predetermined common standard, the stored updated augmented EHR constituting the most current augmented EHR until the Patient receives a subsequent instance of Healthcare delivered by a Provider; and the computer system of the Aggregator automatically communicating a copy of the stored augmented EHR for each identified Patient in a reply to a request from the computer system of any Provider, thereby enabling the requesting Provider to obtain the augmented EHR for each identified Patient with a single request communicated to the computer system of the Aggregator.
