The light formation part 20 includes: a base member 60; semiconductor light-emitting elements 81, 82, 83 mounted on the base member 60; lenses 91, 92, 93 mounted on the base member 60, the lenses 91, 92, 93 converting the spot size of light emitted from the semiconductor light-emitting elements 81, 82, 83; and light-receiving elements 94, 95, 96 mounted on the base member 60 and disposed between the semiconductor light-emitting elements 81, 82, 83 and the lenses 91, 92, 93 in the emission direction of the semiconductor light-emitting elements 81, 82, 83, the light-receiving elements 94, 95, 96 directly receiving the light from the semiconductor light-emitting elements 81, 82, 83. |
