PROBLEM TO BE SOLVED: To provide a form having a bending line in which a mark is more inconspicuous than a case where the mark is formed at a portion to be an outside, in a state in which the form is bent along the bending line.SOLUTION: Relating to a form 10, a right arrow mark 21 and a character string 22 of a "valley-fold for bonding" are formed as a mark 20 which specifies a setting state to an image formation device and, especially, a transportation direction and orientation of front and rear surfaces of the form 10, on a first attaching portion 13a and a second attaching portion 13b which overlap each other in a state in which a bending line 17 is bent by valley folding not to appear outside, being a front side 10a of the form 10.SELECTED DRAWING: Figure 2 |
