According to an embodiment of the present invention, a method for providing a smart consent form for card issuing by a card company server comprises the steps of: receiving a card issuing request from a user over a phone or the Internet; transmitting a URL address connected to a web page for making a smart consent form to a user terminal; receiving a smart consent form including individual the confirmation on the individual items and a user signature from the user terminal; transmitting the smart consent form to a public repository server, and receiving the smart consent form sealed with a time stamp; and transmitting the sealed smart consent form to the user terminal. Thus, manpower and expenses required for follow-up management of a real consent form can be reduced. |
