An electronic care management system comprises a provider data store (60) that stores details of a plurality of providers (2,3,4) registered with the electronic care management system. A consumer data store (40) is configured to store consumer records for a plurality of consumers. A care management planner (51) is configured to process user input to a) define care plans in the consumer records, each care plan having at least one of a health condition and a health goal; and b) define a care team comprising one or more of the providers for assisting the respective consumer in respect of each care plan. A care plan monitor (52) is configured to process inputs related to progress in respect of each care plan and to update respective consumer care plans to include data indicative of progress in respect of the care plans. A care plan manager (53) is operable by a provider of the plurality of providers to input selection criteria in respect of the care plans for which the provider is a member of the defined care team in order to select a subset of consumers and to generate a message relevant to the input selection criteria.
