Provided is an index forming tool capable of forming index portions which can be torn off by hand so that the index portions are aligned at a predetermined position. A index forming tool (100) in which a cutting blade (20) biased upward is attached on a base board (10) comprising a guide path (11) and a slit (12), wherein the tool is provided with a guide portion (33) for positioning a corner of a sheet (2), the cutting blade is pressed down while the guide portion (33) abuts the sheet to thereby form perforations (3) to tear off the corner of the sheet (2), and a region sectioned by the perforations defines an index portion (1). |
