A computer implemented medical records system and method having a mobile computer device that comprises a camera and a communication module, wherein machine-readable instructions connect the device to network and access a structured narrative located on a server, the said structured narrative is displayed on the mobile computer device and the structured narrative comprises a plurality of narratives pertaining to a patient, each narrative comprises an originator identity, a date, and a data item, wherein one data item is selected from one of the plurality of narrative comprising a hyperlinked medical image, whereby the structured narrative pertaining to a patient is stored at a centralized server for maintaining medical records and for communication among medical professionals. |
