PROBLEM TO BE SOLVED: To provide a drive transmission device that can be arranged in a small space and can ensure sufficient strength of extension parts, and an image forming apparatus including the drive transmission device.SOLUTION: A screw driving shaft 63 as a first rotational axis is attached with a first coupling 161 that is connected to a second coupling as a second connection member that is attached to a shaft end part of a screw shaft as a second rotational axis. The first coupling 161 has a plurality of driving pawl parts 161c as extension parts extending in the axial direction formed at intervals in the direction of rotation.
