PROBLEM TO BE SOLVED: To more efficiently create a document having necessary information input thereto.SOLUTION: A portable terminal device 1 acquires document specification information from an IC tag of a document and acquires personal information, such as the address and name from a user ID card such as an IC card driver's license. The portable terminal device 1 transmits the document specification information and personal information acquired on the portable terminal device 1 side to an image forming apparatus 2 by short-range radio communication. The image forming apparatus 2 includes a document database that has document specification information on documents and layout information on an entire image and entry columns associated to each other and registered therein. The image forming apparatus 2 searches for an entire image of a document corresponding to the document specification information and the layout information on the entry columns acquired from the portable terminal device 1, and creates and prints a composite image obtained by inserting corresponding personal information into the positions of the entry columns on the entire image shown in the layout information.SELECTED DRAWING: Figure 1
