<P>PROBLEM TO BE SOLVED: To provide a form with a card which is a card enabling ink jet or laser printer printing and being highly durable as in scuff resistance in printing, and to provide a manufacturing method thereof. <P>SOLUTION: The form with a card has, on at least one side of form paper sheet 101, a printing card 11 having a first printing area 41 peelable via a pseudo adhesive layer 13 and printer-printable on a surface, and a second printing area 43 printer-printable on the surface of the form paper sheet 101. In the form with the card, a first printing part 31 to be printed in the first printing area 41 and a second printing part 33 to be printed in the second printing area 43 are provided on the same side and printed at the same time by the same printer.
