The present invention relates to an albumin conjugated temperature-sensitive and pH-sensitive multi-block copolymer, to a preparation method of the same, and to a long-term sustained release drug delivery carrier using the conjugated copolymer. More particularly, the present invention relates to a conjugate which combines a polyethylene glycol-poly(amino urethane) (PEG-PAU) or polyethylene glycol-poly(amino ester urethane) (PEG-PAEU) multi-block copolymer with albumin; to a preparation method of the same; and to a long-term sustained release drug delivery carrier using the conjugate, which can reduce initial excessive release and can enhance affinity with drugs. The multi-block copolymer of the present invention has a repeat unit represented by chemical formula 1 and has a number average molecular weight of 10,000 g/mol to 25,000 g/mol.
