Aspects of the present disclosure generally relate to systems and methods for accessing, managing, and exchanging patient information across a plurality of providers that may have various discrete medical record systems, including, but not limited to medical facilities, clinics, ambulatory emergency health systems, and other providers (e.g., heterogeneous healthcare providers). In various embodiments, the system receives a request from a requesting party for one or more records of a particular patient. In response, in one or more embodiments, the system identifies what information (if any) the requester is authorized to receive, retrieves the information the requester is authorized to receive from various discrete medical record systems, then normalizes and aggregates the retrieved information. In a particular embodiment, the system also denormalizes the data (e.g., converts the data to the local format and terms of the requester) before transmitting the retrieved data to the requester.

1. A computer-implemented method for updating one or more patient records comprising the steps of:
receiving, by at least one processor, real-time patient data from a computer system associated with a first vendor, wherein the patient data comprises:
information associated with a particular patient; anda first vendor identifier; determining whether the received patent data is included in an existing patient health record database; in response to determining the received patient data is not included in the existing patient health record database, adding, by at least one processor, the received patent data to the existing health record database; in response to receiving the received patient data, locating, by at least one processor, one or more third-party patient registries that include health record data associated with the particular patient; in response to locating the one or more third-party patient registries that include health record data associated with the particular patient, determining whether the one or more third party patient registries include the received patient data; in response to determining the one or more third party patient registries include the received patient data, determining whether the received patient data included in the one or more third-party registries originated from the first vendor based at least in part on the first vendor identifier; and in response to determining the received patient data included in the one or more third-party registries did not originate from the first vendor, modifying, by at least one processor, the received patient data included in the one or more third-party patient registries to include the first vendor identifier.
