PROBLEM TO BE SOLVED: To provide a delivery slip, easily usable by reducing the use number of sheets.SOLUTION: A copying type multilayer delivery slip has a plurality of slips on an upper surface of an envelope part by including a delivery slip 16 in the envelope part formed as the envelope part 20 having an opening 24 on one side short edge in a state of projecting the tip so as to be extractable from the opening, by pseudo-bonding the upper-lower peripheral edges of envelope upper paper 21 and envelope lower paper 22, and a cutoff perforation line is formed along the inner edge of an adhesive part of the left edge and the upper-lower edges so as to be usable by cutting off the envelope upper paper 21 as a sticking slip and delivery destination memo 17. Various shapes can be adopted for the cutoff perforation line. |
