A drum brake device (11) is provided with: a wheel cylinder (15) for spreading a pair of brake shoes (25, 27) disposed inside a brake drum (29) by pressing the adjacent ends on one side of the pair of brake shoes (25, 27) with the tip of a first piston (39) and the tip of a second piston (41) using hydraulic pressure; an extension mechanism (21) that is disposed inside the wheel cylinder and drives the first piston (39) and the second piston (41) in directions away from each other; and an electric actuator for rotating the extension mechanism (21).
