A second region (12) composed of a flat surface is formed at the periphery of a first region (11) having a lens function, and a third region (13) is formed between the first region and the second region. On the surface of the first region, a protection film (24) is formed. The first region is protruded from the second region, and the third region is formed over the entire periphery of the first region. The surface of the third region is retracted from a virtual curved surface (11a), which is formed by extending a curved surface along the surface shape of the first region to the second region. Thus, the protection film having a surface shape which substantially accords with the surface shape of the first region is formed entirely over the first region.
