PROBLEM TO BE SOLVED: To accurately recognize even a code represented in colors including an intermediate density, by forming a code for representing more information within a smaller code area. SOLUTION: An image forming apparatus 100 includes: a storage means 109 storing intermediate density definition information associating colors of an intermediate density, an angle formed by causing first parallel line tint of a first color material to cross second parallel line tint of a second color material, a first ratio for the first parallel line tint to occupy a unit region, and a second ratio for the second parallel line tint to occupy the unit region correspond to one another; and a write unit 104 for falsely representing the colors of the intermediate density by printing the first parallel line tint and the second parallel line tint in the first ratio and the second ratio, respectively, within an area corresponding to the unit region of a printing medium, so that the first parallel line tint crosses the second parallel line tint at the angle.
