PROBLEM TO BE SOLVED: To provide a motion guide device and a roller screw capable of preventing outbreaks of skew even if there is an inclination error in a fitting surface and an offset load is applied. SOLUTION: This motion guide device includes: a track member 1; a moving member 2 having a plurality of load roller rolling surfaces 2d facing a plurality of roller rolling surfaces 1b and a plurality of non-load return passages 8 extending in parallel with the plurality of load roller rolling surfaces 2d and a plurality of direction change-over passages 6 connecting the plurality of load roller rolling surfaces 2d and the plurality of non-load return passage 8 to each other; and a plurality of rollers 3 arranged in a plurality of roller circulation passages. In at least one roller circulation passage of the plurality of roller circulation passages, a plurality of rollers 3 in two line or more are arranged per roller circulation passage. When the moving member 2 is moved in the longitudinal direction of the track member 1 relative to the track member 1, two lines or more of the plurality of rollers 3 circulate in the roller circulation passage by rolling between the roller rolling surface 1b of the track member 1 and the load roller rolling surface 2d of the moving member 2.
