PROBLEM TO BE SOLVED: To provide a suspension board with circuit that positions an optical waveguide and a near-field light generating part with higher accuracy, and reliably radiates light transmitted in the optical waveguide by the near-field light generating part. SOLUTION: The suspension board with circuit 1 includes: a circuit board 2 including a metal support board 11, a base insulating layer 12 formed on the above, and a conductor pattern 13 formed on the above; and an optical waveguide 19 provided at the circuit board 2. A positioning mark 50 and a protection layer 28 covering it are provided at the optical waveguide 19.
