The present invention is to provide a movable form for a concrete structure to easily construct a cap concrete structure on the concrete caisson using the movable form and simultaneously reduce equipment use and construction costs by suppressing the usage of a crane, and a cap concrete structure construction method using the same. The movable form for the concrete structure according to the present invention comprises: a pair of rails disposed alongside each other on both sides of the top surface of the concrete caisson at a fixed interval; and a mold with a transport unit moving along the pair of the rails.
