Implant apparatus and methods directed toward treating conditions involving the knee joint and the patella specifically are disclosed. Full range of motion of the knee joint and tissue integrity are maintained in treatment approaches involving implanting a joint surface load reducing implant proximate the joint to change the direction of the tendons or muscles exerting forces on the joints.

1. A knee joint implant, comprising:
a patellar tendon engaging portion, the patellar tendon engaging portion configured to engage a patellar tendon throughout a full range of motion of a knee joint; and a tension redistribution and contact force manipulation portion configured to alleviate pain associated with patella misalignment; wherein the patellar tendon engaging portion comprises a first chamber containing a fluid and a second chamber in fluid communication with the first chamber.
