A plurality of Bragg gratings (9) are formed at prescribed locations in a laminate having a mounting substrate (3), a cladding layer (22) upon the mounting substrate (3), and an optical material layer (23) upon the cladding layer; channel optical waveguides (16) are formed so as to include each Bragg grating; a mask covering an area corresponding to the grating elements is formed upon the optical material layer; and a side surface (1a) and end surfaces (1b) are formed in each grating element (1) as a result of etching the optical material layer and the cladding layer.
