A form (1) for carrying sensitive information such as a personal identification number (PIN) in a secure manner has an upper lamina (2) and a lower lamina (3) releasably secured together with adhesive (4) arranged to fill perforations (8) in the upper lamina (2). The adhesive (4) in the perforations (8) forms pin heads on the lower lamina (2) when it is separated from the upper lamina (3) to reveal information printed on the form (1) in the area of the perforations (8). The information revealed by the pin heads may be a PIN applied to a pre-determined region of the form (1) such that the PIN is concealed until the lamina (2, 3) are separated. |
