<p><P>PROBLEM TO BE SOLVED: To provide a health data collection device allowing higher-quality health management while maintaining a simple user interface. <P>SOLUTION: A doctor terminal 1 of this health data collection device adds voice data that are voice or the like of a doctor inputted by a microphone 16, or image data that are an image or the like of the doctor inputted by a CCD camera 17 to question data, transmits them to a center server 3 on a communication network 4, and makes the center server 3 store them. A patient terminal 1 downloads the question data, and the voice data or the image data added thereto, and notifies a patient by a liquid crystal display device 18 or the like. A patient terminal 2 adds voice data or image data of the patient to answer data to the question data, transmits them to the center server 3, and makes the center server 3 store them. The doctor terminal downloads the answer data, and the voice data or the image data added thereto, and reproduces them by a liquid crystal display or the like 33.
