A medical skin applicator apparatus comprises an outer housing 534 defining a longitudinal axis, a fluid housing 506 having a fluid chamber for storing a medical agent, a manually engagable member 530 connected to the fluid housing 506 and extending beyond the outer housing 534, and an applicator 504 coupled to the housing 534. The manually engagable member can be depressed to cause longitudinal movement of the fluid housing 506 to the actuated position. The fluid housing 506 has a cap 508 with a penetrable wall to permit access to the fluid chamber and release of the medical agent. The applicator 504 includes an applicator frame and an applicator surface for applying the medical agent to a patient. The applicator frame has an internal penetrating member 558 adapted to penetrate the penetrable wall of the fluid housing cap upon achieving a predetermined coupled relation of the fluid housing 506 and the applicator 504, to thereby permit the medical agent to be dispensed from the fluid chamber and applied to the patient with the applicator surface.
