PROBLEM TO BE SOLVED: To provide a continuous label form not folded at an unintended part. SOLUTION: The continuous label form has a laminate structure where upper and lower base materials having self-adhesive layers are integrally stacked, slip parts surrounded with a cut auxiliary line being arrayed in a continuous direction in the upper base material, and folding lines straight at predetermined intervals in the continuous direction. A side of the cut auxiliary line surrounding the slip parts and extending in the width direction of the continuous label form is constituted by a straight line inclined by 0.10 to 0.40 degrees with respect to the folding line.
