<P>PROBLEM TO BE SOLVED: To provide a sheet with an extracting slit capable of preventing the occurrence of breakage in an outer portion of the extracting slit in extracting an inner portion of the extracting slit in the sheet, and to provide a device for forming the sheet and a method thereof. <P>SOLUTION: The sheet 2 formed by a method for extracting sheets using a sheet punch-out device 1 includes a partially discontinuous first extracting slit 20 of nearly an annular from. The sheet 2 further includes a second extracting slit 22 located at an outer peripheral side or inner peripheral side of the discontinuous portion 21 in the first extracting slit 20. A slit length L2 of the second extracting slit 22 of the sheet 2 is equal to or longer than a length L1 of the discontinuous portion 21.
