The system, comprises a data repository, comprising historical health records and socio-economic condition records of a plurality of patients, and a plurality of processing subsystems that are operationally coupled to the storage device, wherein the plurality of processing subsystems comprise two or more service modules that are configured to provide selective stakeholder services to one or more of a plurality of stakeholders based upon a plurality of parameters.
