PROBLEM TO BE SOLVED: To provide a W/O type cream pharmaceutical preparation containing an agent having a tertiary amine in the structure in the range of PH of 3-6 in which viscosity of the preparation is suppressed from decreasing under not only a high temperature condition but also a low temperature to secure viscosity stability in low temperature storage and high temperature storage so that a cream agent type can be held, and which is excellent in spreadability on the skin.SOLUTION: A W/O type cream pharmaceutical preparation in the range of PH of 3-6 comprises: (A) an agent having a tertiary amine in the structure; (B) one kind or more selected from fatty acid ester of glycerin and polyglycerin fatty acid ester which are ester compounds of a C12-22 fatty acid and a (poly)glycerin; (C) polyoxyethylene hardened castor oil having a polyoxyethylene addition mol number of 5 to 20; and (D) a higher alcohol that is liquid at 25°C.
