This disclosure describes systems, devices, and techniques for determining discrepancies in medical information. In one example, a computer-implemented method includes receiving clinical documentation related to a patient encounter, receiving one or more selected codes associated with the patient encounter, and determining, by the computing device, one or more suggested codes associated with the patient encounter and based on clinical documentation. The method may also include determining one or more discrepancies between the one or more selected codes and the one or more suggested codes, responsive to determining the one or more discrepancies, generating, based on the one or more discrepancies, a query that indicates the one or more discrepancies and solicits user input resolving the one or more discrepancies, and outputting the query for display. |
