In the determination means, a type in which a reference clear vision region of a progressive refractive power lens is made to be equal or narrower is selected when the prescribed fixation disparity amount is at or above a reference fixation disparity amount, whereas a type in which the reference clear vision region of the progressive refractive power lens is made to be equal or wider is selected when the prescribed fixation disparity amount is smaller than the reference fixation disparity amount.
