PROBLEM TO BE SOLVED: To obtain information for evaluating the effect of a request about the use of charging equipment when the request is notified to a user.SOLUTION: A questionnaire processing apparatus comprises: a questionnaire target extraction unit extracting user terminals of users the charging equipment utilization situations of whom correspond to a request about the use of the charging equipment from among a plurality of user terminals of users to each of whom the request is notified; a questionnaire transmission unit transmitting questionnaire information for each of the users of the user terminals extracted by the questionnaire target extraction unit to answer a questionnaire as to whether the user obeys the request consciously, to the user terminals; and an incentive allocation unit extracting user terminals to which operation related to an answer to the questionnaire is input from among a plurality of user terminals receiving the questionnaire information on the basis of contents of operation by the users to the user terminals receiving the questionnaire information, and allocating an incentive to the user of each of the extracted user terminals.
