PROBLEM TO BE SOLVED: To provide a wrapping-integrated delivery slip in which information indicating an article supply source on a surface is visible just after wrapping and which is capable of facilitating wrapping work. ! SOLUTION: The wrapping-integrated delivery slip includes: a slip base material 2 with which the article supply source is printed in any other portion than a delivery slip 2a and an affix slip 2b; and a support base material 6 affixed to a rear side of the slip base material 2. The slip base material 2 and the support base material 6 are connected via a pseudo adhesion layer (an exfoliation layer 3 and an adhesive layer 4) which can be exfoliated and are difficult to be adhered again in a portion of the delivery slip 2a, adhered via a viscous layer 8 that can be exfoliated and re-connected, within a range of a predetermined width from sides 2m and 2h, and adhered via a strong adhesion layer 11 that is difficult to be exfoliated, in the other portion.
