FIELD: medicine.SUBSTANCE: abdominal cavity is sized longitudinally and transversally at 3 levels. The upper level is found within an apex of a right cupula of diaphragm; the middle level is against the middle of a hernial orifice, and the lower one - on a line connecting anterior superior iliac spines. That is followed by measuring a height of the abdominal cavity from the upper to middle levels and from the middle to lower levels that makes it possible to calculate a preoperative volume of the abdominal cavity by formula. The derived value is added to a hernial sac volume to produce a changed volume of the abdominal cavity after the hernial sac contents have been immersed into the abdominal cavity. X-ray examination and sonography are performed to determine how much the abdominal cavity is expected to resize transversally at the middle level after the hernial sac contents have been immersed therein, which is used to determine how long the anterior abdominal wall is supposed to be; a difference of the produced and initial lengths of the abdominal cavity is added to the hernial orifice size to derive a value that expresses a diastasis of aponeurosis borders, which is actually the prosthetic implant size.EFFECT: improving the outcome of the surgical management of ventral hernias, reducing a recurrence rate, preventing compartment syndrome by an individual approach to the prosthetic implant sizing for the purpose of hernia repair.2 dwg, 1 ex
