PROBLEM TO BE SOLVED: To provide a form with a return envelope capable of reducing the time required for a recipient to return and suppressing a manufacturing cost.SOLUTION: A front-and-back pair of a first rectangular piece of paper 1A and a second rectangular piece of paper 2A are bonded by the peelable adhesion of an adhesive layer 12 provided at a joint margin 10 of the first rectangular piece of paper 1A and a peeling layer of the second rectangular piece of paper 2A. A return envelope is formed by folding the first rectangular piece of paper 1A, from which the second rectangular piece of paper 2A is removed, so as to seal a confidential paper surface 1b. Since a recipient can easily form the return envelope, improvement in the rate of return can be expected.
