Disclosed is a form transfer apparatus. According to the present invention, the form transfer apparatus comprises: a base frame formed in the direction of the ground; a plurality of moving parts which is joined together at intervals on the base frame and each bottom of which has a wheel; an elevation frame formed on one side of the base frame to be extended upward; and a chain block joined to the top of the elevation frame and arranged with a connection part to be fastened to a form. The form fastened to the connection part is separated from the ground by the chain block to be supported in the base frame. According to the present invention, the form transfer apparatus significantly reduces a time for joining and separating the form and a form transfer apparatus by fastening the form transfer apparatus to the form using a connection part; and stably and rapidly transfers the form by making the form be supported in the base frame to be transferred. |
