Herein a mouthpiece (2) for use with a handheld inhaler is disclosed. A housing (4) of the mouthpiece comprises an inlet (6), an outlet opening (8), and a channel (10) extending through the housing (4) connecting the inlet (6) and the outlet opening (8) for directing an aerosol from the inlet (6) to the outlet opening (8). An anterior portion (18) of the housing (4) is adapted to be placed in a mouth of a patient. The anterior portion (18) of the housing (4) is configured to direct the outlet opening (8) upwardly in the oral cavity when the anterior portion (18) is placed in the mouth of the patient. Other embodiments disclose an outlet opening (8) placed closer to the upper side (14) of the housing (4) than to the lower side (16) of the housing (4). Further, an inhaler comprising a mouthpiece is disclosed.
