<p><P>PROBLEM TO BE SOLVED: To provide a foldable delivery slip that can prevent outflow of information that is required confidentiality and an accident caused by peel off from commodity. <P>SOLUTION: A first delivery slip 101 has: a first area 110 where first information 111 is recorded on a first surface, and a second area 120 where the adhesive G is applied on the second surface to enclose the first area 110, and has a perforation M in the boundary of the first area 110 and the second area 120. A second delivery slip 102 has: a third area 130 that has a window 131 to expose the adhesive G applied to the second area 120 in the folded state to a fourth surface side; and a fourth area 140 which continues from the third area 130 to the first area 110 in the folded state and is positioned at an inner side than the first area 110 and in which the second information 141 is recorded in the fourth surface, and has the perforation M in the boundary of the third area 130 and the fourth area 140. <P>COPYRIGHT: (C)2013,JPO&INPIT</p> |
