The questionnaire management server 2 includes: receipt information acquisition means 201 for acquiring merchandise names included in a receipt image obtained by photographing a receipt 8 on which merchandise names are printed by the user terminal 7; investigation object merchandise determination means 202 for determining whether or not an investigation object of a questionnaire is included in the merchandise names; first terminal position determination means 203 for determining whether or not the user terminal 7 which has transmitted the receipt image is present in a specified area; questionnaire object determination means 204 for, when the investigation object of the questionnaire is included in the merchandise names, and the user terminal 7 is present in the specified area, determining the user terminal 7 as a questionnaire object; and second terminal position determination means 205 for determining whether or not the user terminal 7 which has transmitted a reply to the questionnaire is present in the specified area.
