PROBLEM TO BE SOLVED: To provide a document information input system capable of accurately inputting alphabetic information corresponding to each item described in an entry document.SOLUTION: A system for inputting alphabetic information corresponding to each item described in an entry document as registration information on a customer database comprises: a scanner for reading a readable information code 17 having information on at least a plurality of items and the layout of each item thereof and arranged at a determined position on the entry document by a predetermined method; a control part for acquiring at least layout data on the basis of the information in the information code 17 read by the scanner and approximating input fields 25, 25 and so on of each item to an entry document on a display to arrange and display them. |
