The purpose of the present invention is to provide a rehabilitation device that can improve minute manipulation and power of concentration together with the dexterity of the hands and arms of a patient requiring rehabilitation treatment. For the purpose, the present invention provides an upper limb and cognition rehabilitation device comprising: a main body part having installation grooves arranged at both sides in a widthwise direction; rail parts traversing both sides of the main body part in a widthwise direction, inserted and fixed into the installation grooves and having rail grooves formed in a lengthwise direction; and a sliding part installed to traverse the main body part in a lengthwise direction and including guiding protrusions at a lower part to be inserted into the rail grooves and move along the rail grooves.
