An endoscopic surgical device which allows easily obtaining images that the surgeon wants and which has high operability, and an outer sleeve, are provided. The outer sleeve is provided with a slider inside of an outer sleeve main body which guides the endoscope and the surgical instrument in the body cavity. An endoscope connection unit and an surgical instrument connection unit are provided inside of the slider, and the outer sleeve has an insensitive zone in which forward and backward movement of either the endoscope or the surgical instrument is not linked to movement of the other, and a sensitive zone in which forward and backward movement of either the endoscope or the surgical instrument is linked to movement of the other. The outer sleeve main body is configured to allow rotating the surgical instrument about the axis thereof without altering the positional relation of the endoscope and the surgical instrument relative to the outer sleeve main body. |
