PROBLEM TO BE SOLVED: To provide a recording form creation device capable of easily creating a recording form to be used for managing the quality of a product for each product.SOLUTION: The recording form creation device includes: an input part 31; and a control part 33 for generating a recording form to be used for managing the quality of a product on the basis of an input from the input part 31. The recording form includes at least basic attributes having product identification items for identifying the product and measurement items indicating subjects related to the measurement of the product corresponding to the basic attributes and input items to which the measurement results of the product corresponding to the measurement items are input. When the product identification items and the measurement items are respectively set on the basis of the input from the input part 31, the control part 33 generates the recording form on the basis of the set product identification items and measurement items.
