A lens plate and an illumination lamp which are not affected by heat from a light source even if the lens plate is small. The lens plate for an illumination lamp has the lens part covering the light source, and the lens part is formed in such a way that a concentric lens pattern is separately formed on an outer central part and an inner peripheral part so as not to be overlapping.
