<P>PROBLEM TO BE SOLVED: To provide a developer supply device capable of more uniformly conveying and supplying developer through the use of a traveling-wave electric field, and to provide an image forming apparatus capable of forming good images by effectively suppressing uneven density. <P>SOLUTION: The developer supply device (5) includes: a developer case (51); and a developer conveying body (54). The developer conveying body (54) is configured to convey the developer (T) along the surface (54a) thereof by forming the traveling-wave electric field. The developer conveying body (54) is arranged facing a supply object (56) so as to supply the developer (T) from the surface (54a) to the supply objects (56 and 41).
