摘要 A surgical system for performing gastrointestinal procedures is disclosed. The system includes an elongated body defining an endoscope lumen. The body is adapted to be inserted into the esophagus with a proximal end extending from a body orifice. A working member includes at least one side disposed suction cavity and a cutting device configured to excise a surface layer from the tissue that is captured in the suction cavity. One or more needles are also provided in the working portion, which can be used to apply suture to the captured tissue and/or to inject fluid into the captured tissue prior to excision. Particular applications may be to treat Barrett's esophagus, to treat GERD, or for performing gastroplasty in the stomach.

A system for performing a gastrointestinal procedure comprising: an elongated body having a longitudinal axis, the elongated body defining an endoscope lumen and constructed and arranged to be inserted into the esophagus with a proximal end extending from a body orifice, wherein the body defines a distal end and a working portion proximal to the distal end; at least one elongated side disposed suction cavity in the working portion for capturing tissue; at least one cutting device positioned in opposing slots in side walls of the suction cavity for resecting an elongated surface layer from the captured tissue portion so as to produce an elongated section of exposed tissue; at least one needle adapted to be passed into the captured tissue portion, the needle having a sharp end portion opposite a back end portion and a main body that spans between the sharp and the back end portions, the needle defining a slot; a shuttle having a first position wherein the shuttle contacts the needle at the back end portion and the shuttle having a second position wherein the shuttle contacts the needle at the sharp end portion, the shuttle releasing the needle when moving from the first position to the second position to advance the needle into the captured tissue portion; and a longitudinally operable locking bar having a first recess adjacent a second recess, the first recess is sized to engage the slot on the needle to lock the needle in a desired position, and the second recess is sized to accommodate the main body of the needle to unlock the needle to enable movement of the needle.
