PROBLEM TO BE SOLVED: To provide an optical waveguide type diffraction grating and an optical module that are capable of reliably operating a diffraction grating formed in a ferroelectric crystal substrate, and thus improving diffraction efficiency and also arbitrarily performing modulation.SOLUTION: An optical waveguide type diffraction grating 10 includes: a ferroelectric crystal substrate 11 having an optical waveguide 11g in which a core shape thereof is of ridge type and an optical path length adjustment part 11p and a polarization inversion periodic structure part 11r are provided; insulation layers 12 disposed so as to hold the ferroelectric crystal substrate 11 therebetween; optical path length adjustment electrode parts 18 which are disposed opposite to each other on the outside of the insulation layers 12 and apply a voltage to the optical path length adjustment part 11p to generate an electric field; and diffraction grating electrode parts 20 which are disposed opposite to each other on the outside of the insulation layers 12 and apply a voltage to the polarization inversion periodic structure part 11r and to generate an electric field. |
