According to the present invention, a fermentation device for a mushroom culture medium comprises: a rectangular support frame; a cylindrical fermentation tank installed to be rotatable in the support frame, forming an entrance in which cotton waste is inputted and a spiral cotton waste development path on the inside, and inputting and discharging water; a bearing support unit supporting the fermentation tank and supporting the rotation of the fermentation tank when rotating; a fermentation tank rotation unit rotating the fermentation tank by being interlinked with a rotation gear formed on one side surface of the fermentation tank; an air supply unit supplying air into the fermentation tank; and a fermentation tank separation prevention unit installed on one side surface of the fermentation tank to prevent separation of the fermentation tank. The present invention can automatically manufacture the culture medium for cultivating mushroom as the cotton waste for the culture medium for cultivating mushroom is inputted to the entrance and the fermentation tank is rotated and driven. Accordingly, the culture medium for cultivating mushroom can be mass produced by nearly one person workforce so that labor productivity is significantly increased and the culture medium for mushroom can be manufactured all-year round, and an output is increased. The present invention can contribute farm income.
