PROBLEM TO BE SOLVED: To provide a concrete sample molding form 50 that may be easily removed, has a small weight and high strength, and may be easily manufactured, and to provide a concrete sample molding form 50 that may effectively release heat generated in curing a test piece (namely, a concrete sample) and requires neither a special structure nor a special tool for removal. SOLUTION: A concrete sample molding form 50 is formed in the shape of a closed-end cylinder including a cylindrical tubular part 51 and a bottom part 52 closing a lower end of the tubular part 51. The tubular part 51 has a multilayered structure at least including a paper layer 10 made of paper and a metal layer made of a metal material.
