PROBLEM TO BE SOLVED: To provide a resin composition for optical waveguide and a resin film for optical waveguide formation, and an optical waveguide using them which can form an optical waveguide excellent in transparency, heat resistance, environment reliability and toughness with high productivity and workability. SOLUTION: A resin composition for optical waveguide formation, and a resin film for optical waveguide formation using the resin composition for optical waveguide formation, and an optical waveguide using them include (A) a polymer having a carboxyl group, (B) urethane (meth)acrylate, (C) a chemical compound having two or more epoxy groups in a molecule and (D) a radical polymerization initiator.
