PROBLEM TO BE SOLVED: To provide a technique for controlling an input content output destination per medical report.SOLUTION: In a medical information input device 6 for creating medical report document data, medical report document data includes a plurality of items to which values can be input, and setting information used for controlling output of the input values in the respective items is configured to be freely set. A data creation start unit 11 acquires a user's creation command to create the medical report document data. An input screen creation unit 11 creates an input screen that includes a value input area to which the values corresponding to the respective items in the medical report document data are input and setting information input area to which the setting information for the input values is input in response to acquisition of the creation command by the data creation starting unit 11. A storage unit 13 stores the setting information on a plurality of item values input on the input screen created by the input screen creation unit 12 in association with the medical report document data.
